据《暗黑破坏神3》总监Josh Mosqueira透露，《暗黑破坏神4》最初的设计理念与最终成品大相径庭。它并非我们熟知的核心动作RPG玩法，而是更接近于《蝙蝠侠：阿卡姆》系列的动作冒险体验，并融合了Roguelike机制。
这一信息来自彭博社记者Jason Schreier新书《Play Nice: The Rise and Fall of Blizzard Entertainment》的节选，近期发表于WIRED网站。书中，多位《暗黑破坏神》团队成员回顾了从《暗黑破坏神3》到《暗黑破坏神4》期间的事件。由于《暗黑破坏神3》被认为是暴雪的失败之作，Mosqueira希望在《暗黑破坏神》系列中创造出全新的体验。
当时，该项目代号为“Hades”，由少数艺术家和设计师与Mosqueira共同构思了《暗黑破坏神4》的早期版本。这个版本采用的是肩视角，而非传统的等距视角。战斗方面，类似于《蝙蝠侠：阿卡姆》系列，更加注重动作性和打击感。更值得注意的是，游戏引入了永久死亡机制，角色死亡即为彻底死亡。
尽管Mosqueira获得了暴雪高层对大胆尝试不同《暗黑破坏神》作品的支持，“诸多因素”最终导致团队无法将这款Roguelike风格的《暗黑破坏神4》变为现实。其中，雄心勃勃的阿卡姆风格合作多人游戏元素带来了巨大的技术难题，设计师们开始质疑：“这还是《暗黑破坏神》吗？”设计师Julian Love 沉思道：“操作方式不同，奖励不同，怪物不同，英雄不同。但它很黑暗，所以它还是一样的。”此外，暴雪开发团队逐渐意识到，这款Roguelike风格的《暗黑破坏神4》实际上将成为一个与《暗黑破坏神》系列完全不同的全新IP。
《暗黑破坏神4》近期推出了首个大型资料片“憎恨之器”。“憎恨之器”将玩家带到邪恶的纳汉图领域，时间设定在1336年，深入探讨了大罪恶之一墨菲斯托及其对庇护之地的险恶计划。
